The Health Office is open M-F, from 8 AM until 2 PM. In the event of illness, a student must report to the Health Office. All students must sign in and out for every visit to the Health Office. All visits are documented and the Family Rights & Privacy Act ( FERPA ) Regulations as well as state regulations will be strictly adhered to. All communication, written and/or verbal, is confidential. Information will be shared only with those who need to know. Should the information be of a sensitive nature, the parent/guardian will be consulted prior to disclosure. Lack of current Health Information will delay or impede the delivery of timely health care, should a situation arise.
A current physical exam is required at the start of each and every school year.

Any student diagnosed with Asthma/Reactive Airway/Respiratory Problem that requires the administration of prescription medication for control of this condition must submit this form. NJ State Law.

Allergies Requiring Emergency Medications
Any student diagnosed with an allergy that requires Emergency Medication (Epi-Pen/Epi-Pen Jr./Auvi-Q) must submit:
1. Emergency Health Care Plan
2. Authorization To Administer Epinephrine
3. Acknowledgement Form School Policy on Epinephrine

Medication to be Administered During the School Day
Any medication that will be administered during the school day must be accompanied by the Medical Authorization Form. Page 1 of this form is a two part consent. Parent completes the first part, and physician completes the second part. Full directions are on page 2.
