Media Release Form

This form explains potential uses of student photographs, video images, and names by Academy of the Holy Angels and allows you to grant or deny permission to Holy Angels to release them for display or publication. Holy Angels uses various media to highlight the Holy Angels experience. For example, student images may be published in printed materials, videos, AHA school websites, AHA social media websites in accordance with the Social Media Policy. Yearbook and class photos are handled separately. If you do not want your daughter to be in the class photographs or yearbook, please contact the Yearbook Moderator directly via email at [email protected]. This release does not apply to the school yearbook. Parents have two options: • Parents may grant permission for their child’s image and name to be published. Select this option if you permit your child’s photograph and name to appear in school publications, on the Holy Angels website, on Holy Angels social media pages and released to external organizations such as in press releases to the media. • Parents may deny permission for any display or publication of their child’s image and name by the Academy of the Holy Angels. Select this option if you do not want your child’s photograph and/or name to be used on the Holy Angels website, social media, or printed publications, or in release to external organizations such as in press releases to the media. This consent form remains valid throughout your child’s enrollment at Academy of the Holy Angels or until a new form is completed and signed by a parent / guardian. This form replaces any previous authorization.

By entering your name and the date, you electronically acknowledge: I am the student’s parent, legal guardian, an international student's responsible adult, or authorized to sign for the student. I have read and understood the above document. This form was completed accurately and to the best of my knowledge.
